Dr. Geoffrey Keyes, our Los Angeles, CA double board-certified plastic surgeon, will carefully examine your nasal and facial structure and provide an in depth consultation to discuss your goals and determine the best treatment to achieve an excellent result. His usual surgical approach is through closed rhinoplasty, which means he does not make an incision on the outside skin of the nose. Everything is done from within the nostril. He is able to perform rhinoplasty using this technique because of his dual board certification in otolaryngology or ear, nose and throat surgery and plastic and reconstructive surgery. Rarely, an open rhinoplasty technique is performed when necessary because of severe problems related to a previously performed nasal operation by another surgeon.

"This review may be a little early as I just got my cast off my nose today...but I am over the moon. I don't know how Dr. Keyes does it, and I'll admit I was skeptical while the cast was on as the cast and tape can distort things. I was actually a nervous wreck. But when that cast came off today, it reduced me to happy tears. He did everything I wanted and more. My nose is now straight, and it looks amazing. Even swollen I love it! I can breathe through my left nostril for the first time and it feels so good. I seriously feel like a new person. I'm so excited to see how it progresses over the next year. Dr. Keyes was the only consultation I had, and for a second it crossed my mind, did I jump the gun on this? Do I need a second opinion? Something in my gut trusted him. And I'm so glad my gut was right. His reviews and his before and after photos on his website speak for themselves. I am so thankful to Dr. Keyes and Kimberly. They're so easy to reach and they care about you just as much before the surgery as they do after. They are wonderful. Couldn't recommend them more. I am so happy. Thank you Dr. Keyes and Kimberly. From the bottom of my heart!"- S.B. / Yelp / Nov 05, 2019
"I started considering rhinoplasty when I was 16, I am 28. I hated my nose, the shape and the size. I tried for many many years to love as it was. But to be honest, I always knew one day I would take the step. Los Angeles has a big offer of plastic surgeons, and is hard to know who is good and who is the best. After some months of research, I narrowed my options and had two consultations with two different surgeons. From the very first second I knew Dr.keyes would be my surgeon. I am a really hypochondriac person, and Dr.keyes and Kimberly made me feel confident, safe and comfortable during the whole process. The day of the surgery I was really nervous and everyone made it really easy for me. Post op is painless, just uncomfortable. Dr Keyes and Kim were available for me 24/7in case I had any question or concern. That really makes a difference. I am one month post op and even swollen, I looooove my nose, it changes your life. It is natural, fits my face and I could not be any happier. If you are thinking about having a rhinoplasty, Dr.Keyes and Kimberly are the team you are looking for, thank you both!!"- S.R. / Yelp / Feb 04, 2020
"Rhinoplasty - I'm scheduled to have rhinoplasty with dr Geoffrey Keyes in LA. I had my consultation and I had my pictures taken and then photoshopped by the dr to show me what he can do with my nose. He is nice and very straight forward and gives his honest opinion on what realistically can be done. I told him I want a really small Barbie nose . He said he can give me that but it also all depends on what happens in the OR , but I told him to go as small as my body will allow. I'm also interested in lip lift and fillers so I'm going to call tomorrow to see if that can be done as well."- M. / RealSelf / Feb 02, 2020
"I know my surgery is recent but I’m happy I picked them over others I consulted with. - First and foremost Dr. Keyes and Kimberley, his surgical assistant, are the best. As soon as I went in for my Consultation they both made me feel comfortable. Dr. Keyes is well regarded in the rhinoplasty society. When I got my surgery done on June 22, 2020 I was super nervous but Kimberley was there by my side keeping me calm. Dr. Keyes did exactly what I wanted on my nose. Kimberley is great in staying in touch with you seeing how your healing and if you’re ok. I know my surgery is recent but I’m happy I picked them over others I consulted with. I would definitely refer him in a heartbeat."- G. / RealSelf / Jul 01, 2020
"A little over 2 months post op and have to say I’m loving the results so far! Everyone says my nose fits my face and looks better than before. My nose kept changing over these past 2 months because I’m so early in my recovery. It is however, looking better over time. Dr. Keyes and Kimberly are very nice people and are good at answering any questions you may have. The day of surgery I started panicking and got anxiety. Granted, this was my first time being put under general anestesia. Dr. Keyes, Kimberly, and the anesthesiologist made me feel at ease. I finally feel confident enough to take pictures and not be self-conscious of others looking at my nose! Lol"- V.P. / Yelp / Apr 07, 2022
Both women and men seek consultation for cosmetic nasal surgery. If you're displeased with the overall look of your nose and have specific concerns you wish to enhance or repair, this surgery may be the recommended answer. It is very important for the expectations of the patient are clearly understood to achieve the best surgical result.
Once you are determined to be a candidate, we will discuss how to accomplish your surgery. There are various techniques and approaches that Dr. Keyes implements during rhinoplasty. Prior to undergoing a procedure, Dr. Keyes will talk with you about which surgical method he intends to use and explain what he would be doing to your nose. He will be educating you on the surgical technique and answer questions that you may have about rhinoplasty. This will also simply take the mystery out of your surgery. Our goal is to make you feel prepared and confident with the long-lasting alterations that we will be making to your nose
Rhinoplasty surgeries are performed in a local facility on an outpatient basis. Dr. Keyes prefers to utilize the closed rhinoplasty technique (also known as endonasal rhinoplasty) when performing nasal surgery. The reason many surgeons do not use the closed rhinoplasty technique is that the technique requires a more advanced, complex approach to reveal the composition of the nose and internal structures. Closed rhinoplasty is performed from within the nostrils using a variety of surgical tools and state-of-the-art instruments. Incisions are made from within the nose so scarring isn’t visible. This lack of a visible scar is important to many patients.
Open rhinoplasty means that an incision is made along the soft tissue between the nostrils, making the misshaped cartilage more visible. Many doctors believe that using the open technique allows them to alter and manipulate the contour of the nose with heightened precision and control. It is important to remember that either technique can be used, but it is the skill of the surgeon that determines the result. Exposure of the tip cartilages allows them to be sculpted. The goal, in most cases, is to make the nasal tip look more refined and have an adequate degree of projection. Slight rotation of the nasal tip helps to soften the appearance of both the frontal, or forward-looking view, and the profile. Advantages of this technique include a single, camouflaged incision is created and the preservation of facets is maintained. Successful tip plasty surgery is crucial to the improvement of the appearance of the nose. For boxy-tipped noses, Dr. Keyes offers nasal structure reshaping, dome division, and cartilage grafting. For crooked, misshaped noses may perform septoplasty, a nasal hump reduction, an alar base reduction, turbinate reduction, tip plasty, or a cartilaginous spreader with plumping, and tip grafts.
What to Expect
After your nose surgery, you will be monitored closely in a private recovery room to be monitored by one of our registered nurses. Once the effects of the anesthesia have worn off, Dr. Keyes will release you to recover at home. Dr. Keyes will call the night of surgery to check on you and to make arrangements for your first follow up visit. The area around the nose will be very swollen and bruised for a week or so. In almost all patients, there is bruising beneath the eyes in addition to the nose. Cold compresses and prescription medication can assist with these effects. Your post-operative appointment with Dr. Keyes will be scheduled within 48 – 72 hours after surgery to assess your healing progress. On average, it takes around 2 – 4 weeks to heal from nose surgery. Within this process, strenuous activity or exercise should be avoided.
Dr. Keyes will call the night of surgery to check on you and to make arrangements for your first follow up visit. You will typically come in for a check-up the day after your rhinoplasty. You will have additional follow-ups at one week, two weeks, and at one, two, and three months. Dr. Keyes proudly provides complete post-operative care for life. You can schedule as many check-ups as you want at no additional charge. Typically, you will be able to see improvement in the shape of your nose immediately after your dressing is removed at your one-week postoperative visit. You should expect the initial post-surgical swelling to last at least six weeks with final swelling subsiding over the course of a year.
Frequently Asked Questions
How much does rhinoplasty cost?
The cost of rhinoplasty varies based on what technique is used and how complex your procedure is. Before estimating costs, Dr. Keyes will listen to your concerns and goals to help develop your custom treatment plan. When this is done, he can discuss costs for the facility, anesthesia, and more. For your best results, it's important to focus on selecting an experienced surgeon, like Dr. Keyes, instead of the lowest cost. To help make your surgery easy to afford, Geoffrey R. Keyes, M.D., F.A.C.S. accepts several payment methods and our office can help you find low-interest financing.
Will my insurance cover functional nose surgery?
Although rhinoplasty for cosmetic reasons is not paid by insurance, functional rhinoplasty that is medically necessary to improve your breathing may be covered. If you may be applying for reimbursement from your insurance, please tell Dr. Keyes in your consultation. We can help you submit any needed information and paperwork to your insurance to process your claim.
Are there nonsurgical options to change my nose?
If you want to smooth lumps and bumps on your nose, a wrinkle filler (like Restylane or Juvéderm) might be an option. A filler can temporarily improve minor imperfections, which might help you decide to get a rhinoplasty. Nose surgery is better for larger changes, long-lasting results, and to adjust the internal structure for better function. At your consultation, Dr. Keyes can tell you more about nonsurgical nose reshaping.
What about ethnic rhinoplasty?
Ethnic rhinoplasty is about improving the nose while still keeping your cultural identity. This is often a concern if you are African-American, Asian, or Middle Eastern. In your consultation with Dr. Keyes, you can talk about what you want to keep and what you want to change. This will allow him to personalize your treatment plan to give you your best results.
Can I combine nose surgery with other facial surgeries?
Some patients think about combining rhinoplasty with another procedure, like cheek or chin augmentation. Having multiple surgeries for a dramatic result can be reviewed during your consultation with Dr. Keyes. Once he understands your goals, he can help you choose which procedures will be in your custom plan. Rhinoplasty can help your whole face appear more balanced, so you should wait to see your results before getting another procedure.
